- Diagnostic and x-rays – 100%
- Periodic exams 2 times per year
- All x-rays including pano 1 every three years
- Preventative – 100%
- Child dental cleaning 2 times per year
- Adult dental cleaning 2 times per year (with no periodontal disease)
- Fluoride and sealants
- SAVE 10% on all other procedures!*
*Implants, Fillings, Crowns, Dentures and partials, Oral Surgery (extractions only), Root canals. Includes dental implant placement and crown restorations, as well as implant-secured dentures.
Dental Savings Plan Guidelines
Your dental benefits are honored only at NWA Family Dental. This is a discount, not an insurance plan. These benefits are not valid at other dental offices. Your dental savings plan cannot be used with another dental plan. Worker’s compensation, referrals to specialists, hospitalization, hospital charges, and dental costs covered under medical or auto insurance are excluded. Fees are non-refundable. Premiums or refunds will not be issued at any time if the participant decides not to use their savings plan. Patient’s responsibility of dental costs are due at the time of service.
